Fischione Instruments customers use our products for the investigation of a wide range of materials across a broad spectrum of market segments. We recognize our customers as being experts regarding their materials, so we rely on those skills and knowledge to eliminate any possible hazards when returning products to our facility.
The Declaration of Decontamination form gives us an understanding of what our products may have been exposed to during use. We request that the primary end-user complete this declaration, which certifies that the product returning to our facility does not pose a health risk; this ensures the health and safety of both our employees and our suppliers.
Before requesting an RMA number to return merchandise to Fischione Instruments, fill out the Declaration of Decontamination form and then email it to rma@fischione.com.
